Good Health Statement Questionnaire
This feature allows Carriers to present a good health statement questionnaire to the insured before they sign a policy to ensure there have been no health changes since the client first applied. Good Health Statement questions and answers can be mapped to a Carrier form.
Carriers also have one of two options when implementing the Good Health Statement feature:
1. Prevent consumers from eSigning the policy package if they answer “Yes” to a question on the Good Health Statement Questionnaire. Good Health Statements answered with a “Yes” answer will create a DocFast change request and stop the delivery process and change request notification emails will be delivered to the appropriate parties.

2. Allow the consumer to continue to the eSignature page even after answering a question with a “Yes” answer.
The Good Health Statement Questionnaire is an optional configuration for a Carrier and can be switched on or off on a per-policy basis. There are costs associated with the implementation of this feature.
